HOW TO GET STARTED - 2 SIMPLE STEPS

1. Reserve your rebate before making your purchase.  Fill out the "Part 1: Rebate Reservation" form and submit it.  You will receive a "Part 2: Rebate Confirmation" form, and you will then have 30 days to complete your purchase and submit your "Part 3: Rebate Request Form."

2. Make sure the model you plan to buy is a qualified ENERGY STAR appliance.

• ENERGY STAR DISHWASHER
• ENERGY STAR CLOTHES WASHER
• ENERGY STAR REFRIGERATOR

**Special Note: You will be required to submit a copy of your appliance store receipt or invoice along with your Part 3: Rebate Request as a proof of purchase.  The receipt or invoice must show the appliance make and model number, store location and date of purchase.  Save your paperwork!!
